• Physical Therapists will assess the patient’s physical condition and establish a physical therapy plan that will help that will help patients achieve maximum independence with their activities of daily living and minimize joint and muscular pain. Training for safe ambulation, transfer, and gait balance will be provided.
• Occupational Therapists will work with patients in achieving independent living goals, through exercise programs and adaptive equipment.
• Speech and Language pathologists will assess, diagnose, and treat those individuals with speech and language difficulty, problems swallowing and cognitive-communication disorders.

Therapeutic services are an essential part of our home health care program. Our licensed therapists focus on restoring function, improving mobility, and enhancing communication abilities.

Physical Therapy
Our physical therapists assess your physical condition and develop a customized therapy plan. These exercises and training techniques are designed to reduce joint and muscle pain, improve movement, and enhance independence. Patients also receive training on:

Occupational Therapy
Occupational therapists work closely with patients to help them regain skills needed for independent living. Whether you need help using adaptive equipment or need a specialized exercise routine, we’re here to help you succeed in your day-to-day activities.

Speech Therapy
Our speech-language pathologists assess and treat individuals with speech and language difficulties, swallowing issues, and cognitive-communication disorders. With patience and expertise, they help patients regain clarity, communication skills, and confidence.

• Registered Nurses and Licensed Vocational Nurses evaluate, assess patients medical needs and render treatment, instruction and teaching in accordance with the Plan of Care, which is duly approved by the attending physician.
• Medical Social Worker will assess your psychosocial needs and long term planning goals.
• Registered Dietitians will asses your nutritional needs and provide plans for your specific diet.

The cornerstone of our home health services is the Plan of Care, which is designed and regularly updated in collaboration with your attending physician. Our experienced nursing team includes Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs), who:

• Conduct comprehensive health assessments

• Monitor vital signs and chronic conditions

• Administer medications and treatments

• Provide patient education and family instruction

• Offer post-surgical care

Our nurses work closely with patients and families to ensure every aspect of care is understood and effectively managed at home.
